How do I deal with a child in my care who has severe development issues? ex. 4 years old and on the bottle?

She has no comprehension when you explain she has done something wrong. Throws a tantrum when she doesn't have her bottle (which she was mad and threw behind the bureau). Refuses to drink from anything but the bottle. Is abusive to my own child. And seems to have no comprehension of rules or timeout. Any help would be greatly appreciated
